Notice to clients
Your records, and where a complaint goes.
I’m Acelli. Texas asks me to tell you two things in writing: how to get a copy of your records, and where to take a complaint. Both are on this page, in my words first and then in the state’s.
Getting a copy of your records
Write to me at [email protected] and tell me which you want: an electronic copy, a paper copy or a summary. If you ask for an electronic copy, I’ll send it within 15 business days, and a paper copy or a summary comes within 30 days. I may charge a reasonable, cost-based fee. Everything else the law says about your record is on my Privacy practices page, under Your Rights, if you want it.
If you ever need to complain about me
The state board that licenses me takes complaints about counselors. Its notice is below, in its own words, and under it is how to reach the board.
Notice to clients
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. Please call 1-800-821-3205 for more information.
Texas Behavioral Health Executive Council, 1801 Congress Ave., Ste. 7.300, Austin, Texas 78701. (512) 305-7700, or the 24-hour complaints line, 1-800-821-3205. bhec.texas.gov.
Consumer complaints, the kind about how I run my business rather than my license, go to the Texas Attorney General, at texasattorneygeneral.gov.
A complaint about your privacy can go to the federal privacy office too. You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-800-368-1019, or visiting https://www.hhs.gov/hipaa/filing-a-complaint/index.html.
Whichever of these you use, I won’t hold it against you.
Questions about any of this
